Unemployment Benefits Fraud: Forms 1099-G

Any Commonwealth employee who has received an unexpected or potentially erroneous Form 1099-G may be the victim of unemployment fraud.

Criminal enterprises using stolen personal information from earlier national data breaches have been attempting to file fraudulent unemployment claims through the Department of Unemployment Assistance (DUA) system. This is part of a national unemployment fraud scheme.

If you believe someone has applied for unemployment benefits using your personal information, report the suspected fraud to DUA using this secure online form or call (877) 626-6800. DUA will send this information to law enforcement, review your report, and send a corrected Form 1099-G. Additionally, take steps to protect yourself using this information provided by DUA.
