CTR Releases new Electronic Funds Transfer (EFT) Authorization Agreement Form
The Office of the Comptroller is pleased to offer a new Electronic Funds Transfer (EFT) Authorization Agreement Form, available for immediate use on the CTR Intranet.
The new EFT Authorization Agreement Form is available in a fillable PDF format to ease transaction processing in a remote work environment.
Additionally, the form includes:
- A new certification section for state agencies.
- A new section for VCC and VCM Document ID#.
- A voided check or bank letter is required for new enrollments or change enrollments.
Clear explanations are provided for each required field, for both the vendor/account holder, as well as for state agencies. Any questions about the new form can be directed to [email protected]. Older versions of the EFT Authorization Agreement Form will no longer be accepted after sixty (60) days.
PLEASE NOTE: Due to the ongoing elevated risks to the Commonwealth’s financial systems posed by remote work throughout state government, CTR is instituting a five-day pause period after receipt and before finalizing any EFT account change requests.
